General Survey Drawing Rules

  1. HOW TO ENTER: Registered panelists of the BizRate.com Online Research Panel are automatically entered into the prize drawing by completing and submitting online a survey by the expiration date. If a panelist does not want to complete and submit a survey online, the panelist may enter the sweepstakes by mail. To do so print name, address, city, state or province, zip or postal code, and complete email address, on a 3 1/2"x5" piece of paper, insert in an envelope and mail to: BizRate.com Online Research Panel, (NAME OF SURVEY) Drawing, 12200 W Olympic Blvd, Suite 300, Los Angeles, CA 90064, which must be received by the expiration date. Limit one entry per panelist per survey/drawing. No responsibility is assumed by Sponsor, and entrants waive all claims for lost, late, illegible or misdirected surveys and mailed entries or for any problems of technical malfunction of any telephone network or lines, computer equipment, software, failure of any email service or any combination thereof including without limitation any damage to entrant's or any other person's computer resulting from participation in this promotion. All surveys become the property of the Sponsor. Any attempt to damage the BizRate.com Online Research Panel survey site or undermine the legitimate operation of the promotion is a violation of criminal and civil laws. Should such an attempt be made Sponsor reserves right to prosecute violators to the fullest extent allowable by law.
  2. PRIZE: Prizes may vary for each survey. Each prize is payable in U.S.$. United States federal, state & local taxes and any applicable Canadian taxes on prizes are the sole responsibility of the winner. No substitution or transfer of prizes permitted.
  3. PRIZE DRAWING: Winner(s) selected in a random drawing to be conducted on or about the expiration date by independent judges whose decisions are final on issues relating to each sweepstakes. The odds of winning depend upon the total number of eligible entries received. Judging will take place at BizRate.com Online Research Panel, Los Angeles offices. Winners will be notified by email within 5 business days of drawing date. If Sponsor is unable to contact potential winner(s) by email or if any potential winner fails to respond to the prize notification within 14 days of issuance of notification or if any winner fails to comply with any of the requirements, an alternate winner shall be selected through another random drawing conducted by judges. Winners of prizes valued at more than $100 U.S. will be mailed and required to sign affidavit of eligibility/publicity and liability releases, and return same, properly executed within 14 days of notification. Winners consent to the use of their name for promotional purposes, including online announcements, without additional compensation, except where prohibited by law. Allow 4-6 weeks after verification of winner status for the receipt of prize by mail. Canadian citizens: Any litigation respecting the conduct and awarding of a prize in this publicity contest may be submitted to the Régie des alcools, des course et des jeux. Any potential winner who is a Canadian resident must complete a time-limited mathematical skill-testing question administered by telephone or mail in order to be deemed a winner.

    If, in the judges' opinion, there is any suspected or actual evidence of electronic or non-electronic tampering with any portion of the survey and/or sweepstakes, or if technical difficulties compromise the integrity of the survey and/or sweepstakes, the judges reserve the right to terminate the survey and/or sweepstakes and conduct a random drawing to award the prizes using all eligible online and write-in entries received as of the termination date. Should the sweepstakes be terminated prior to the stated expiration date, notice will be posted on the BizRate.com Online Research Panel web site.

  4. ELIGIBILITY: This sweepstakes is open to registered BizRate.com Online Research Panelists who participated in this study. If an individual under 18 years of age is pulled as a winner, their parent or legal guardian is required to sign an affidavit. The prize check will be issued in the parent or legal guardian's name. This promotion is not open to employees of BizRate.com and its affiliates, licensees, franchisees, subsidiaries, advertising and promotion agencies and the immediate family members of each. By entering, participants without limitation, release and hold harmless BizRate.com and its affiliates, licensees, franchisees, subsidiaries, advertising and promotion agencies from any and all liability for any injuries, loss or damage of any kind in connection with this sweepstakes or acceptance and use of a prize won. Void where prohibited.
  5. WINNER'S LIST: The name of the winner(s) will be available after the winner(s) has been officially notified. Postings can be found within Research Panel Services on the BizRate.com Online Research Panel web site (www.bizrate.com/panel/panel.xpml?content=winners). The list may also be obtained by mailing a self-addressed stamped envelope to: (NAME OF PROJECT) Winners, c/o BizRate.com Online Research Panel, 12200 W Olympic Blvd, Suite 300, Los Angeles, CA 90064.

BizRate.com Online Research Panel Registration

  1. To be entered into the "BizRate.com Online Research Panel Registration Drawing" for a chance to win $1,000 cash, you must submit the entire survey and you must enter your email address. Only those panelists signing up for the first time will be entered into this drawing.
  2. This Registration Drawing is sponsored and promoted by BizRate.com.
  3. One entry per participant per survey/drawing. No purchase necessary to enter. In fact, we will never try to sell you anything.
  4. The Registration Survey Drawing will be held once every three months. One winner will be selected from registrants who submit a completed Panel Registration Survey during the entry period, as illustrated in chart below:

  5. If you do not wish to complete a survey to enter, you may print your name, street address, city, state or province, zip or postal code, complete email address and the words, "New Registrant by Mail," on a 3˝"x 5" piece of paper, place it in an envelope and mail to: Online Research Panel Registration, Drawing C/O BizRate.com, 12200 W Olympic Blvd, Suite 300 Los Angeles, CA 90064, to be received by the date in chart above.
  6. This sweepstakes is open to registered BizRate.com panelists who participated in this study. If an individual under 18 years of age is drawn as a winner, their parent or legal guardian is required to sign an affidavit. The prize check will be issued in the parent or legal guardian's name.
  7. The winner will be selected from among all eligible responses and posted on our site after the winner has been officially notified.
  8. The winner will be notified via email or by United States Postal Service. The sponsor assumes no responsibility for late, lost or misdirected notifications. Winner will be required to sign an affidavit of eligibility/publicity and liability and Federal and State tax releases, and return same, properly executed within 14 days of notification. Failure to return documents as specified, or if prize notification is returned as undeliverable, the entry will be disqualified, and an alternate winner selected.
  9. If the winner does not respond to the affidavit within 14 days, that winner will be disqualified and the Sponsor will randomly draw an alternate winner.
  10. If the winner is a Canadian resident, the winner must complete a time-limited mathematical skill-testing question administered by telephone or by mail in order to be deemed a winner.
  11. Allow 4-6 weeks after drawing date for receipt of prize by mail.
  12. The odds of winning depend upon the total number of eligible entries received.
  13. The prize, if cash, will be paid by check in U.S. dollars.
  14. By accepting a prize, the winner agrees to the use of his/her name in publicity and advertising in this and similar promotions without additional compensation, except where prohibited by law.
  15. All federal, state or provincial and income taxes on prize, if any, are the sole responsibility of the prize winner.
  16. All surveys become the property of the Sponsor.
