In my most recent order an item was missing. I looked at the packing slip and saw that you were out of stock and did not charge me for it. Instead of making me wait for the rest of my order, you sent the remainder of the order.

I may need to order in multiples of ten for kidney and liver disease tests and would like assurance that you will be able to meet my orders (with reasonable notice) as i do not want to disappoint my customers.

iN COMPLETING THE QUESTIONS, i AM PRESENTLY UNAWARE OF WHAT PRODUCT i ACTUALLY ORDERED. I'd have to go home to see what product I ordered. If the product was identified on the Questionnaire, then I could accurately answer
